Supply Difficulties Expected with MMRV

Merck & Co., Inc. has notified the American Academy of Pediatrics and the Centers for Disease Control and Prevention (CDC) that, based on current market demand, the company projects the existing supply of ProQuad, used to vaccinate against Measles, Mumps, Rubella and Varicella (MMRV) is likely to be depleted by July 2007.

Once depleted, ProQuad is not expected to be available for the remainder of 2007. Vaccine providers should begin transitioning from ProQuad to MMR and Varivax according to their current supply of vaccine. In its April issue, AAP News reported that Merck (Whitehouse Station, N.J.) had temporarily suspended production of bulk varicella, which is used in the preparation of its three varicella-containing vaccines, due to lower than expected yields in recent production lots. Varicella bulk is stored frozen until it is needed in the final preparation phase of each varicella-containing vaccine. Thus, the lower virus yield in the production of bulk varicella zoster virus (VZV) does not affect the quality of any of Merck’s VZV-containing vaccines currently on the market, nor does it affect any lots of vaccine manufactured and ready for release to the market.

Production of VZV bulk will remain suspended until Merck identifies the cause of the lower-than-expected virus yield. Yellow Immunization Card




Immunization Registry

The Los Angeles Orange Immunization Network (LINK) is an Internet-based system used by medical providers and other approved agencies in Los Angeles and Orange Counties to track the immunizations of their clients. LINK is a partnership between the Los Angeles County Department of Health Services, the City of Pasadena Public Health Department,  the City of Long Beach Department of Health and Human Services, and the County of Orange Health Care Agency.

LINK can help to:

  • Retrieve immunization records of clients seen by another provider without calling to request the information.

  • Eliminate duplicate immunizations with rapid access to current, complete immunization histories.

  • Highlight critical client information such as reaction histories and contraindications.

  • Display instantly and accurately vaccine doses due at each visit, based on current recommendations. Forecast each client’s upcoming vaccine due dates to keep them on schedule.

  • Streamline charting by automatically recording manufacturer and lot number of each dose in the client’s record.

  • Replace lost California Immunization Records (yellow cards) by printing a new copy with up-to-date information.

  • Keep track of vaccine inventory.

  • Generate data needed for HEDIS reporting, vaccine usage reports, and immunization rates for your practice.

  • Ease the burden on office staff by generating the reminder/recall notices.

Vaccines for Children (VFC) Program 

The VFC program began on October 1, 1994 and represents an unprecedented approach to improving vaccine availability nationwide by providing vaccines free of charge to VFC-eligible children through public and private providers.

The VFC program:

  • Provides public purchased vaccines for eligible children at no charge to public and private providers in the United States, the Commonwealth of Puerto Rico, the Virgin Islands, American Samoa, Guam, and the Commonwealth of the Northern Mariana Islands. 

  • Automatically covers vaccines recommended by the Advisory Committee on Immunization Practices (ACIP) and approved by the Centers for Disease Control and Prevention (CDC).

  • Saves parents and enrolled providers out-of-pocket expenses for vaccines.

  • Eliminates vaccine cost as a barrier to immunizing eligible children. 

  • Reduces the practice of referring children from the private sector to the public sector for vaccination, keeping children in their medical home for comprehensive health care.

Vaccine Handling and Storage

Don’t forget to keep your vaccines cool! A vaccine must be kept within a specific temperature range; keeping some vaccines too cold or not cold enough may damage their integrity and effectiveness.

 

To protect your vaccines and keep them viable, remember to:

  1. Check that the thermometer in your refrigerator and freezer registers recommended temperatures. Use this temperature range guide to help you.

  2. Record temperature of each vaccine refrigerator and freezer every morning and before closing. You can download a temperature log and keep it in a folder or binder that you keep next to the refrigeration unit for easy access. You will need Adobe Acrobat Reader to view this page.

  3. Do not store vaccines in the refrigerator door. Opening and closing the door exposes vaccines stored in the door to temperature fluctuations that may damage the vaccines.

  4. If vaccines need to be transported, transport in insulated containers with  ice packs but do not let the vaccines touch the ice pack. NOTE: it is recommended to not transport the Varicella vaccine.

  5. Rotate vaccines weekly to ensure vaccines with the nearest expiration date are used first.

  6. Put a "do not unplug" sticker on or near the outlet connected to the vaccine refrigerator and clearly label the circuit breaker of the vaccine refrigerator so that it is not accidentally turned off. Free stickers can be ordered from the Pasadena Immunization Coalition Material Order Form  

 

  1. Vaccine that must be kept refrigerated (stored between 35° to 46° F or 2° to 8° C): DTaP, DTaP-Hib, DT, Td, Hep B-Hib, Hib, IPV, Hepatitis A and B, and PCV.  It is recommended that you "Aim for 40" degrees in your vaccine refrigerators to prevent your refrigerator temperature from dropping too low.

  2. MMR is a vaccine that may be kept refrigerated (stored between 35° to 46° F or 2° to 8° C) or frozen (keep at 5° F or -15° C or colder).

REMEMBER: THE LOWER THE TEMPERATURE, THE COOLER IT GETS. For example, 30° F is colder than 35° F and -18° C is colder than -15° C.

  1. Varicella vaccine must be kept frozen (keep at 5° F or -15° C or colder).

Vaccine Information Statements (VIS)

Federal Law requires physicians to provide patients or the child's parents/legal guardians with current Vaccine Information Statements (VIS) prior to providing certain vaccinations.

VIS are developed by the CDC and are thoroughly studied by panels of experts to ensure accuracy. VIS are meant to be supplemental information for a person receiving a vaccination or for the patient's parent/legal guardian. However, they are not intended as a substitute for interaction with a healthcare provider.

Most adverse reactions from a vaccine are very mild; risks of serious vaccine consequences are much less likely than the risks associated with not receiving a vaccination. Nevertheless, complications are possible and it is important to provide the patient or parent/legal guardian with impartial information on vaccine safety and the diseases that vaccines protect against.

As a healthcare provider, you are required to do the following:

  1. Before administering vaccines, a healthcare provider must provide the patient or     patient's parent/legal guardian with a copy of the most recent VIS available for the vaccine. Make sure the patient or parent/legal guardian has time to read the VIS prior to vaccine administration.

  2. Record in the patient's Immunization Record and History Form the date VIS was given.

  3. Record in the patient's Immunization Record and History Form the VIS publication date which appears on the bottom right hand corner of the page. It is crucial that you give the most current VIS.

Vaccine Adverse Event Reporting System (VAERS)

You can now submit your patient's adverse reactions towards a vaccine using a secure online from. You will need Netscape Navigator 6.0 (or higher) or Internet Explorer 5.0 (or higher) in order to view and send the needed information.

Objective scientific information about immunizations and other resources, such as "talking points" sheets can be obtained from the National Network for Immunization Information in a resource kit for healthcare providers entitled "Communicating With Patients about Immunizations." "Talking points" are structured to help conduct a brief education session that will improve parents' understanding of the recommended vaccines, including their benefits and potential risks as compared to the risk of the diseases they prevent. California School Immunization Law For Healthcare Providers - mandates that children must be properly immunized prior to entering school or childcare. Parents must present their child's immunization record to school or childcare staff as proof of immunization prior to admission. Healthcare providers are required to give or update the parent’s copy of the child's Immunization Record whenever these immunizations are administered. Children who have not completed all immunizations can attend school or childcare as long as they are currently up-to-date (no doses currently due) on their immunizations and continue to receive subsequent immunizations to remain up-to-date on their immunizations.

Annual Awards of Excellence in Immunization Practices

Every year, the Pasadena Immunization Coalition hosts an event honoring the hard work and commitment invested by local healthcare providers to protect and improve the health of all residents, especially young children who are the most vulnerable from vaccine-preventable diseases.  The event has a three-fold purpose:

  • to honor local healthcare providers who:

a) have achieved the highest immunization coverage rates in the city      and/or achieved the Healthy People 2010 goal 90% up-to-date for 24-month olds,

b) have demonstrated excellence in patient care by ensuring that vaccines are always accessible to all children who need them, and

c) have implemented outstanding vaccine management systems that safeguard the quality and safety of all vaccines;

  • to provide updates about new policies, innovations, programs and technologies related to immunization through distinguished and reliable speakers;

  • to promote awareness about the importance of timely and appropriate immunization.
